Description
NHS South West London CCG on behalf of all London CCGs is lead commissioner for the Urgent Shared Care Plan (UCP). The UCP is a software service to enable the sharing with urgent care services (111, London Ambulance Service, Emergency Departments in Acute Trusts) of treatment escalation plans, crisis care plans, end of life care plans, patient specific protocols and personalised care plans which are documented by different health and care disciplines.
